Value-Based Care: Sustaining Clinical and Operational Performance Improvement
Hospitals and health systems are committed to increasing value and have taken steps to ensure the delivery of high-quality, patient-centered affordable care. By improving clinical performance and reducing variation in care, among other things, hospitals are delivering on the value equation. This is achieved, in part, through team-based care, real-time data analytics, enhanced payer-provider
collaboration and hardwiring clinical and operational performance improvement. Sustaining these achievements, however, remains a challenge as hospitals and health systems continue the delicate balance between fee-for-service and risk-based contracting. And, as the executive dialogue participants suggest, to truly move the needle, hospitals and health systems, government and community organizations, among others, must address the social determinants of health.
