Mt. San Rafael Hospital is a critical access, 25-bed rural hospital located in the community of Trinidad, Colorado. Mt. San Rafael Hospital treatment facilities include the hospital, emergency department (ED), and two outpatient clinics. Mt. San Rafael Hospital serves a wide geographic area surrounding Trinidad, the southern-most city in the state off I-25 into New Mexico. The hospital’s ED annually serves approximately 11,000 patients of all ages from pediatrics to geriatrics and covers an ambulance district that brings local families from as far away as two hours to receive emergency care. Mt. San Rafael Hospital provides essential emergency treatment, acute care, condition management, and inpatient services needs of the widely-dispersed demographic found in a rural farming and mining community.

The Challenge

In 2015, Mt. San Rafael Hospital was lagging in meeting Meaningful Use requirement deadlines. Making the challenge even greater, Mt.
