Closing the Loop between Chronically Ill Patients and Providers to Reduce Readmissions

Jefferson Community Health & Life (JCH&L) is a 17-bed critical access hospital and family medicine clinic located in the rural community of Fairbury, Nebraska. JCH&L serves an essential healthcare role in this geographic area, providing acute care, family medical care, long-term care, and outpatient care for residents of all ages. Its services include swing bed, rehabilitation, surgery, radiology, obstetrics, laboratory testing, and visiting specialists. JCH&L has been serving the residents of Fairbury since 1963 and employs more than 250 people. Additionally, JCH&L operates 40 long-term care beds and delivers comprehensive family medicine and outpatient care to a service area of more than 7,000 residents.
JCH&L has leveraged the YourCareEverywhere™ (YCE) patient engagement solution and the YourCareCommunity® (YCC) patient portal since the solution was introduced in the community and went live in Spring 2017. In the process of rebranding and meeting Meaningful Use Stage 3 measures, JCH&L recently collaborated with YourCareEverywhere to grow enrollment in their YCE My Wellness Dashboard, increase participation in the Smart Moves Diabetes Prevention Program, and boost awareness and attendance at JCH&L’s 2017 Women’s Health Fair. Using YCE Marketing Services to help design marketing materials for participating employers and their internal employees, JCH&L executed an HRA Campaign and Corporate Challenge using targeted strategies including targeted email programs, audience-specific website landing pages, and challenge communications kits.
As its ongoing mission, JCH&L seeks to inspire healthy living by providing exceptional health and life services to every person, every time. The healthcare industry has acknowledged JCH&L for their consistent delivery of quality medical services and has awarded JCH&L with accolades including:

Despite being recognized as an industry leader for finance, quality, clinical outcomes, and patient satisfaction, in the fall of 2017, JCH&L saw challenges including physician turnover and outmigration.
How could JCH&L create awareness and messaging that targeted their patients, employees, and community to help reverse some of these trends and increase the visibility of its new brand that launched in early 2017?
