The Value of Embracing New Consumer and Patient Engagement Solutions

Case Studies on Managing Health Care and Lifecare from Innovative Hospitals

  • Event Date:
  • Event Time: Noon Central

As health care delivery models increasingly become consumer- and patient-centered, focus on both consumer engagement and patient engagement continues to play a pivotal role in improving care outcomes and reducing health care costs. In this new market, providers will be challenged to offer more information to both patients and consumers and to assist in proactively managing their health and wellness.

In this webinar, learn from two critical access hospitals which are dealing with consumer engagement challenges similar to many other hospitals nationwide. Both hospitals needed a partner who provided solutions and services that could span across both the continuum of care and into consumers' lives.

As part of its rebranding efforts, Jefferson Community Health & Life decided to focus on its mission to inspire healthy living by providing exceptional health and life services for every person, every time. Learn how Jefferson integrated its existing services and patient portal with a consumer engagement platform to engage people where they live, work and play, planting seeds for healthy lives to take root in their community.

Mt. San Rafael wanted to expand its reach beyond the four walls of its hospital to build meaningful, relevant interactions with the consumers and patients in its market. Learn how the hospital has activated consumers in the community, reinvigorated its patient base and raised patient portal usage 40 percent through a consumer engagement solution — including a mobile-friendly personal health data management solution — and the use of wellness tools, communication campaigns and more.

You Will Learn How to:

  • Describe how a powerful combination of content, wellness tools, mobile apps and health records can engage the health-oriented consumer and patient on a continuous basis
  • Evaluate your digital strategy — web and mobile — and your ability to create value for consumers and patients online
  • Create brand loyalty with consumers before the need for care
  • Explain new ways to drive patient engagement to achieve the latest Meaningful Use objectives
  • Expand the value of a hospital outside the walls and into the community with an MU certified platform and mobile app to assemble and aggregate consumers' health care information and provide them with access anytime, anywhere to help them navigate their own care 24/7/365
