Optimizing Resource Effectiveness Across the Care Continuum

  • Event Date:
  • Event Time: Noon Central

Now, more than ever, a strong ambulatory care network is required for organizational success and the provision of high-value care. Organizations must weave together the full spectrum of inpatient and outpatient services to provide seamless care at the right location, with the right resources, at the right time. A System of CARE (clinical alignment and resource effectiveness) is a clinical and business model that provides seamless care and clinical and informational integration across the care continuum.

An Sg2 expert will address relevant trends across the System of CARE, including those impacting the physician enterprise, access points and post-acute care.

As part of a System of CARE framework, value must be optimized at all settings. A Vizient Non-Acute expert will provide insight into the relevance of supply chain management across ambulatory assets as a means of driving down costs and reducing variation. For many organizations, the acute care setting has been the primary point of focus for supply chain management. Often organizations overlook the importance of managing costs across ambulatory settings, which is an untapped opportunity for the reduction in cost of highly utilized services.

Then, we will explore the System of CARE from a clinical perspective. A Vizient’s expert in the use of data for performance improvement will describe the challenges that U.S. health systems have in obtaining and analyzing useful data across the continuum of care, and provide recommendations on what insights currently can be gleaned to develop the appropriate structures and processes. Join this webinar to better understand how to optimize the System of CARE and ambulatory assets at your organization to improve value to the patients you serve.

What You Will Learn:

  • The ultimate need of developing a high-performing care continuum
  • Relevant trends shaping strategic imperatives for ambulatory services
  • How supply chain may be better managed to improve costs and ultimately, value across the continuum
